To confirm you’re the owner of the said phone, you are required to send the following details with your email or written request.
- Full street address
- Phone Number
- Serial Number (IMEI number or ESN) of the phone
- Photocopy of the back of the phone showing the serial number
- Time Frame of phone record
- Sworn Statement with your signature
